HIFU Brow Lift for Heavy or Hooded Brows: What It Can and Cannot Do

Some clients come in and say the same thing: "I look tired, but I am not tired." When we look closer, the reason is often above the eyes. The brows sit a little lower than they did ten years ago. The upper lid looks heavier. In photos, the eyes look smaller than they feel.

HIFU can help some of this, but not all of it. This guide helps you understand what is really making your upper face look heavy, what HIFU can do for the brow line, and when another option is a better fit for you.

Three reasons your upper face looks heavy

Heavy brows are usually not one problem. It is often a mix of three things. When you know which one is strongest for you, it is much easier to decide if HIFU makes sense.

1. The brow has dropped. After 40, the skin and soft tissue of the forehead slowly lose firmness. The brow, especially the outer tail, sits a little lower each year. The outer corner of the eye starts to look more closed.

2. Upper-lid skin folds over. This is what people call "hooded" eyes. Extra skin rests on the lid crease. Some people have this look from a young age because of their eye shape. For others, it comes slowly with age.

3. The forehead works hard to hold the brows up. Many people with heavy brows raise their eyebrows all day without noticing, just to see more clearly. Over the years, this draws lines across the forehead. So forehead lines can be a sign of heavy brows, not only of age. We explain more about why forehead lines show up early in a separate article.

A one-minute mirror test before you decide

You can do this at home. Use a mirror near a window, in natural daylight.

  1. Relax your forehead completely. Close your eyes, count to five, then open them slowly without raising your brows.

  2. Look at where your brows sit. Is the outer tail at the same level as the inner end, or lower?

  3. Look at your upper lid. Can you see the lid crease, or does skin cover part of it?

  4. Now gently lift the outer brow with one finger, only a little. Does the upper lid look clearly lighter?

If a very small lift of the brow makes a clear difference, the brow position is a big part of the problem, and HIFU may help. If your upper lid still looks covered even when you lift the brow, the extra lid skin itself is the main issue. HIFU cannot remove that skin, and it is better to know this before you spend money.

What HIFU can do for the brow line

HIFU sends focused ultrasound through the skin surface and makes small heat points under the skin. The skin then builds new collagen and becomes firmer over time. On the forehead and around the brow, only shallow 1.5–2.0 mm cartridges are used, because the skin there is thin and the bone is very close.

When the skin above and under the brow becomes firmer, it can hold the brow a little better. For many clients, the result is a small lift at the brow tail. That sounds modest. But around the eyes, even a small lift can change how awake you look. The outer corner of the upper lid may also look a little lighter.

What HIFU cannot do for heavy brows

Being clear here can save you money and disappointment.

HIFU does not remove extra eyelid skin. If skin rests on your lashes, or makes it hard to see to the side, please see an eye doctor or a surgeon first. That is not a beauty question anymore.

HIFU does not pull the brows up like surgery. A surgical brow lift can move the brow many millimetres. HIFU gives gentle support to the skin, not a big change in position.

HIFU does not stop the forehead muscle. If you keep raising your brows out of habit, the lines from that movement will still appear. Botox works on the muscle to soften movement lines; HIFU works on the skin. We only mention this so you understand the difference.

HIFU is not for the eyelid itself. The cartridge is never placed on the lid or pointed into the eye socket. It stays on the forehead, under the brow along the bone, and at the outer corner of the eye.

Who is a good fit, and who should wait

A good fit is usually someone around 40 to 55 whose outer brow has dropped a little, whose lid looks lighter with a small finger lift, and who wants a natural change with no downtime. It also suits people who already did HIFU on the cheeks or jawline, and now feel the upper face looks older than the lower face.

Please wait, or choose another way, if the lid skin blocks your vision, if you had eyelid or brow surgery recently, if the forehead skin is inflamed, has acne or a wound, if you had filler in the temples less than 4 weeks ago, or if you are pregnant or breastfeeding.

The session and the weeks after

The forehead and brow are more sensitive than the cheek because thin skin sits directly over bone — clients typically rate it 4–6 out of 10, against 2–4 on the cheek. Each shot is a brief hot prickle, not a lasting ache.

Afterwards: mild redness for an hour or two, possibly slight swelling under the brow for one to three days, occasionally a few days of mild numbness on the forehead. No downtime; makeup from the next day.

Right after the session and for the first 1–2 weeks, the skin feels slightly tighter as old collagen contracts; the real result builds gradually over the following 8–12 weeks as new collagen forms. The result is clearest at month 2–3, once the new collagen has formed. One session typically holds for 12–18 months and then fades gradually with age and skincare habits.

How to get the most from a small lift

A small lift looks best when the rest of your habits help it. Wear sunscreen on the forehead every day, also under a hat. Try not to rub your eyes hard when they feel tired. Notice when you raise your brows while reading your phone, and relax them.

Take a photo before the session, in the same window light as your mirror test. Take another one at month 2–3. Small changes like this are easy to miss in the mirror, but easy to see side by side.

Can HIFU fix hooded eyes?

It can make the outer lid area look a little lighter when a low brow is part of the cause. It cannot remove extra lid skin. If skin still covers your lid when you gently lift the brow, it is better to ask an eyelid surgeon.
